Appointments, Prescriptions and Summary of Care Access Online
The Practice is pleased to provide patients with access to its Clinical System via a Secure Web Interface.
Patients can access appointments, prescription services and also access coded areas of their medical record online. To do this, you need to attend the practice in person with photo identification to obtain a password, this will allow you access to the practice clinical system.
The Practice will take a copy of your passport/driving license as evidence you have requested this service and this will be scanned onto your notes.
Parental access is applicable up to the age of 16 years. Gillick competence will apply if the child understands and requests this.
Warning
Please keep your login details safe, these are your responsibility. This is especially important if you have access to your records. Remember if someone orders prescriptions on your behalf they will be able to view your medical record. If you think someone else has access to your details or feel coerced into viewing these online please let us know, we can help.

About The Oaks Medical Centre
A long-established practice which was originally located on Askew Avenue, Hull, we moved to our purpose-built premises at The Oaks Medical Centre, Hull on 2nd January 2001.
The practice currently has four partner doctors
A large modern building, The Oaks offers a pleasant and welcoming atmosphere with easy access for the disabled. The waiting room is a good size, with comfortable seating and a range of health information leaflets and magazines to hand. We also have an area for small children to play in plus amenities for baby changing and breast-feeding.
